Haas Rotary Repair

Carefully complete the form and submit to the Rotary Repair Service Group by clicking the Submit button at the end of the form. You will be contacted with a Return Authorization (RA) number to be attached to the unit before shipping to the repair facility.

A credit card number or a purchase order will be required to obtain a (RA) for repair. The purchase order will have a maximum of $1,500.00. Should the cost of the repair exceed $1,500.00, you will be contacted with an accurate cost of repairs. Your credit card will not be billed until the repairs have been completed and you have been advised of the cost. Return shipping will be best way ground unless otherwise specified. Every repair comes with a 90-day parts and labor warranty.
